I hereby give permission to Signature Images to use my photos and likeness in all forms and media for advertising, portfolio, demo, trade, stock photography, editorial, altering without restrictions, and all other lawful purposes. 

 

In exchange for the good and valuable consideration received, I grant to Signature Images, LLC, its legal representatives, licensees and assignees, all rights to the photographic image(s) taken for an indefinite time period, for use in all lawful purposes, subject to the terms and conditions described herein.

 

I waive any right that I may have to inspect or approve of the finished product(s) and any copy or other matter that may be used in connection therewith, or to the eventual use that it might be applied, except as noted below.

 

I authorize Signature Images, LLC to use the image(s) for purposes related to the promotion of said business, including, but not limited to: advertising, portfolio, composite cards, exhibitions, contests and promotional internet websites. I do not authorize Signature Images, LLC to sell publication rights to any or all of the image(s), except with my prior, written consent. I release, discharge and agree to hold harmless, Signature Images, LLC, its legal representatives and assigns, and all persons acting under it permissions or authority, from any liability by virtue of any blurring, distortion, alteration, optical illusion or use in composite form, whether intentional or otherwise, that may occur or be produced in the taking of such photographs, or other images or in any subsequent processing or reproduction thereof, as well as any publication thereof (i.e. portfolio), including without limitation, any claims of libel, defamation or violation of rights of privacy or publicity.

 

I hereby warrant that I am of legal age and have the right to contract in my own name, or if contracting for a minor, I warrant that I have the right to contract for the minor. I have read the above authorization, release and agreement prior to its execution, and I am fully familiar with the contents thereof.

 

I understand I am entitled to no compensation. I release the photographer of all forms of claims and liability related to my photo usage. I am over 18 years of age, and I fully understand the terms of this release.

 

This release shall be binding upon the undersigned and his/her heirs, legal representatives, successor and assigns.
